My pain pen is similar to a tens machine in that it gives small electric shock's. I get my PA to give 15 shock's at the top of my buttock then slowly work her way down the root of the pain until she reaches my ankle. I've always found the ankle is important to zap in the right area; don't ask me why but maybe its due to me having full feeling.
Just a word of caution regarding the Pain Pen. I bought one a month or so ago and paid about £30 for it, which I thought was a trifle excessive for what is essentially a slightly re-worked pieizo-electric gas lighter, but today I saw one on-line offered for £109.99 !!!!! Yep, £80 more. So shop around, folks, and avoid
THIS outfit.
Incidentally, it has helped for shoulder, elbow and hand pain. It worked less well for my sciatic pain. I guess my bits are too deeply buried.
