Caffeine gives me problems. UTI's give me problems. Sometimes I will have leakage and then can go weeks with no issues. I'm not on any meds. I believe I have more leakage when I see more blood in the catheter tip. My thought is that any time there is irritation in the urinary tract I am more likely to have bladder spasms and leakage.

If you can reduce any trauma during cathing (use more lube, slow down, maybe use a smaller diameter catheter) and figure out if your diet is adding to the spasms it might help.

Of course there is always a chance that you have developed some stones that are causing the spasms. It sounds like you had a urodynamic study, did they look for stones?
Boat racing accident July 2006. Still hoping for some return!

Do you drink a lot of liquids? If so what liquids? If I drink water I'm ok, if I drink anything with caffeine I have to piss...a lot. The other day I didn't sleep much, so I bought one of those 5 hrs energy bottles on the way to work, they have like 8 oz in them. Within 30 minutes I had to piss, you would have thought I had drank a half gallon with how much I pissed. Within the next 2 hrs I went as much as I normally go in a day and I drink about 60-80 oz of water every day. I never drink stuff with caffeine, no pops, no coffee, no energy drinks. With your problem, the first place to start looking is what might be causing it and what your putting in your body.

It is not unknown for a spastic bladder to grow thicker walls over a period and thereby reduce the size of the bladder. Also, increased spasm for whatever reason will only make it fire earlier.

This scenario may have absolutely nothing to do with your situation but one you could check out with the doctors. Have them check if there is increased pressure on your kidneys, too.

Didn't answer the question of what you drank.
