Help With Hcpcs Codes For Power Wheelchairs

Does anyone have a list of HCPCS codes for power wheelchairs including codes, description and fees?

I've spent a long time at the CMS website (i.e. maze) and came out empty. This is as far as I got: http://www.lamedicar...?articleid=6697 but no descriptions and fees! (note in the link the reference to the ever mysterious "appendix a")
