Voice To Word Document

It's not for a quad or tetra but rather for my wife. She is studying counseling and in this year of study they do role play counseling sessions which are recored onto DVD, she then has to write it up word for word as a transcript and then analyze the session.
For her to play and pause the DVD hundreds of times and type out the transcript is just not going to happen and I wondered if anyone out there uses any software products that would even come close to doing this?

My wife is going to be in a situation where she will always be in the interview but the other person will change and also both of them will be in an ambient setting of a room and not using a microphone very close to their mouthes to ensure maximum clarity.
Maybe what I'm looking for is either ridiculously expensive or not available.
I was hoping that maybe the Quad / Tetra members here may have an idea.

Get your wife to listen to the dvd via headphones and simultaneously dictate what is being said to the voice recognition software. That way it only has to learn her voice. It will take a little longer than if done by some sophisticated voice to document software (if such exists) but will be a lot quicker and easier than typing it all out.
