Have you tried Zanaflex. My husband has severe spasms and he says it works better than baclofen. He takes 30mg 3x a day and i doesn't help. He got phenal injections and that is the only thing that really help, but they wear off in about 6months. Still debating on the baclofen pump. Not sure if that is what we want, but that is what the dr's recommend.

You can take up to about 100mg of baclofen a day I think, but you need to increase it gradually.

80mg is the legal dose in the UK, if your getting real bad spasms then ask to be put on DANTRIUM (Dantrolene) as well as the baclofen, With regards to drivin try crossing your legs and pulling the up to the front of the seat, it may help break them. (the spasms that is not your legs) .Hope this helps.

untill 3 weeks ago i was on 100mg of baclofen - 25mg 4 times a day, and dantrium too.
to be honest, it wasn't doing anything to the spasms.
just had the pump and i'm on 50th of the dose and much more effective.
its each to their own i guess but this is the route i ended up going.
no oral meds now which is a bonus too.

I too am on 100mg daily (along with dantrolene and gabapentin). Spoke to my Brother about this, (he's a consultant in rehabilitiation relating to any nerve damage). Max normal dosage in the UK is according to him 120mg and he favours a reactive approach telling his patients to adjust the amounts and when they take them according to their spasms, take a little more on a bad day, a little less on a good day. So an additional 10mg in the evening shouldn't do any harm (but don't go bumping it up by large amounts). If it works just tell your doc when you next see him. I am not a doctor.

To be honest if your spasms are as bad as you say then you need more than just another 10mg of baclofen and for that you definately need to go back to your doc. If it was me I'd say try the gabapentin first.

I'm going down the pump route later this year. Seems like the way to go.

This is from a site I found and is information from the FDA. I am not a medical professional, but have taken Baclofen for 14 years now, and am using a Baclofen pump at this time.

Baclofen

Dosage Form: tablets

Baclofen Dosage and Administration
The determination of optimal dosage requires individual titration. Start therapy at a low dosage and increase gradually until optimum effect is achieved (usually between 40 to 80 mg daily).

The following dosage schedule is suggested:

5 mg t.i.d. for 3 days

10 mg t.i.d. for 3 days

15 mg t.i.d. for 3 days

20 mg t.i.d. for 3 days
Thereafter additional increases may be necessary but the total daily dose should not exceed a maximum of 80 mg daily (20 mg q.i.d.).

The lowest dose compatible with an optimal response is recommended. If benefits are not evident after a reasonable trial period, patients should be slowly withdrawn from the drug (see WARNINGS Abrupt Drug Withdrawal). (source)
